Study suggests that latrepirdine, which previously failed in clinical trials, may be successful if tested on patients with earlier stages of the disease

An international team of scientists led by researchers at Mount Sinai School Medicine have discovered that a drug that had previously yielded conflicting results in clinical trials for Alzheimer's disease effectively stopped the progression of memory deterioration and brain pathology in mouse models of early stage Alzheimer's disease. The findings, published July 31, 2012 in Molecular Psychiatry, demonstrate renewed potential for this compound and could lead to clinical trials in patients with early stages of the disease.

Latrepirdine, known commercially as Dimebon, was initially sold as an antihistamine in Russia, approved for use there in 1983. In the 1990s, researchers at the Institute of Physiologically Active Compounds in Moscow determined that the compound appeared effective in treating Alzheimer's disease in animals. They continued their research in humans and performed several studies, including Phase I and II trials, all of which showed significant and sustained improvement in cognitive behavior with minimal side effects. The Phase II trials, performed in Russia, were overseen by U.S. Alzheimer's researchers, including Mary Sano, PhD, Director of the Mount Sinai Alzheimer's Disease Research Center.

However, when research was continued in the United States in a Phase III trial, the drug did not demonstrate any improvement in people with the disease, causing the sponsors to halt further clinical study of the drug in Alzheimer's disease. Some researchers have speculated that the Russian patients might have had different disease stage or subtype of Alzheimer's, and therefore were more responsive to treatment than the patients in the Phase III trials in the United States.

Before the failed trials were announced, researchers at Mount Sinai School of Medicine, led by Sam Gandy, MD, PhD, Professor of Neurology, and Psychiatry, and Director of the Mount Sinai Center for Cognitive Health, began studying the mechanism of action behind latrepirdine in the current study, which is supported by the Cure Alzheimer's Fund.

Dr. Gandy's team randomly administered either latrepirdine or placebo to mice engineered to present the early stages of Alzheimer's disease and found that the drug halted both behavioral decline and progression of neuropathology. In evaluating how latrepirdine improved memory, John Steele, PhD, a neuroscience graduate student working with Dr. Gandy, and Lenard Lachenmayer, MD, a postdoctoral fellow working under the supervision of Zhenyu Yue, PhD, Associate Professor of Neurology at Mount Sinai, found that the drug enhanced autophagy, the so-called "self-eating" process of cells that protects the brain from neurodegeneration.

"When we learned that latrepirdine failed in patients in the United States in 2010, scientists around the world were disappointed and perplexed," Dr. Gandy said. "We wanted to find out why the drug did so well in Russia but then showed no effect in the global studies. The findings from our animal model studies indicated that this drug should not be discarded, and that, if its mechanism of action can be optimized, it still has potential."

Dr. Sano points out that not only did latrepirdene have significant and sustained effect in the Russian study but it also showed a mild effect in one study of patients with Huntington's disease.

"Since cognitive benefit is what really matters to patients and families, it is critical that we explore every mechanism by which it might occur," Dr. Sano said.

"While this is just the beginning, our research shows that this previously cast-off drug still has strong therapeutic promise," Dr. Gandy said. "Autophagy drugs are believed to hold great promise for a range of neurodegenerative diseases, and these data raise the question of whether further basic science work on latrepirdine might lead to optimization of the drug so that a more potent drug could be developed, and subsequently tested in human clinical trials.

"This is especially true since we know that latrepirdine is an extremely safe drug and in view of the recent failure of the first key trial of the drug bapineuzumab," Dr. Gandy added. "Also, as may be the case with all amyloid-lowering drugs, initiating latrepirdine trials before amyloid deposition begins may be the key. Now, with the new brain amyloid scans that began at Mount Sinai in June, we can easily establish who those patients are."

Looking ahead, Drs. Gandy, Yue, and their collaborators are planning to test latrepirdine in mouse models of other protein buildup diseases such as Parkinson's disease, Lewy body dementia, and chronic traumatic encephalopathy, the Alzheimer's-like condition athletes endure from boxing, football and hockey. Dr. Sano notes that so few agents show any improvement in cognition that it is critical that to exhaust every potential lead.

Police release photo of Ripken kidnapping suspect

A photo of the man suspected of abducting the mom of baseball hall of famer Cal Ripken Jr. was released today by Maryland cops.

The photo shows a man walking into what appears to be a store wearing a white button-down shirt and an orange and black baseball cap. Police describe the suspect as a white male, approximately 5-foot-10 with short brown hair and glasses.

Vi Ripken, 74, was abducted sometime between 7 a.m. and 8 a.m. Tuesday by a man who used a handgun to force her into her 1998 Lincoln Town Car, according to the Aberdeen Police Department.

She was found unharmed in her car on Wednesday near her home, but was "restrained and confined," according to the Baltimore FBI.

The kidnapper is still on the loose and police believe he is armed "and should be considered to be dangerous."

Earlier today, Baltimore FBI spokesman Rich Wolf told ABCNews.com that investigators were searching for a surveillance camera that may have caught the suspect. He said authorities are looking into surveillance footage and money machines as possible sources of an image of the suspect.

Authorities are also trying to determine whether Ripken's abduction was related to her famous family.

"That's exactly what they're trying to do, find out whether this guy knew who she was before he took her," Wolf said. "We think that will be a key to the case."

Aberdeen police said they "know of no ransom" and Wolf could not say if Ripken's bank account had been accessed while she was being held by the suspect.

Gus Kowalewski, Ripken's next-door-neighbor, told the Associated Press that he spoke to Ripken on Wednesday and she said that the man tied her up, blindfolded her and did not seem to know that she was part of a famous family.

"He said he just wanted money and her car," Kowalewski told the AP.

Kowalewski said Ripken also told him that the man originally planned to put tape over her eyes, but used some sort of mask or blindfold after Ripken told him she was claustrophobic.

"He lit cigarettes for her, they stopped for food," Kowalewski said. "He said, 'I'm not going to hurt you. I'm going to take you back,' and that's what he did."

After the kidnapper left Ripken in her car, she reportedly honked her car horn until a neighbor found her, Kowalewski told the AP.

Kowalewski did not respond to requests for comment from ABCNews.com.

The Ripken family released the following statement, according to ABC News' Baltimore affiliate WMAR:

"As you now know, our mother was abducted at gunpoint from her home yesterday morning. This has been a very trying time for our family, but we are grateful and relieved that mom is back with us, safe and healthy. We want to thank everyone for their tremendous support, especially all of the law enforcement agencies that worked so hard and quickly. This is an ongoing investigation, so we hope everyone understands that we cannot comment further at this time."

Cal Ripken Jr. is a baseball icon who played shortstop and third base for the Baltimore Orioles for 21 years. He is perhaps best known for playing 2,632 consecutive games during his career, breaking Lou Gehrig's record. Ripken's brother Billy Ripken also played for the Orioles, and their father, Cal Ripken Sr., managed them both for a time.

Looking back on high school, I?m often surprised that I graduated with even a modicum of science literacy ? let?s just say that my projects usually seemed a bit rushed, which is to say they were entirely completed the night before the due date. But for the teens who competed in this year?s Google Science Fair, an annual competition that invites teenagers from around the world to?digitally?submit their science projects, I think it?s safe to say that science is a blessing and an opportunity, and not a curse. Among this year?s amazing contestants, grand prize winner?Brittany Wenger, a 17-year-old girl from Florida, seems to be stealing the show ? and with her amazing project, a neural network that can diagnose breast cancer with?99.11% accuracy, can you really blame her?

When it comes to breast cancer detection, the least invasive method, Fine Needle Aspiration (FNA), is often the least accurate. On the prowl for a research project, Wegner thought she might be able to harness the power of computer to boost the diagnostic method?s accuracy.

Wegner, who has been fascinated by Artificial Intelligence since the 7th grade, spent more than 600 hours coding her groundbreaking?artificial neural network. The program, which operates from Google?s Cloud, learns from patterns and mistakes in data sets. After inputting?681 fine needle aspirate samples, her program was able to learn the similarities and differences across the entire data set, eventually ?teaching? the network to detect?cancerous?tumors given a fine needle aspirate with near perfect accuracy. In honor of her outstanding scientific achievement, Google has awarded her?a $50,000 scholarship for college, a lego trophy, a trip to the?Galapagos Islands, and perhaps most importantly, worldwide recognition.

Wegner plans to leave her project online, and encourages hospitals and physicians to input their own data, which will only stand to improve the accuracy of her network. As for her own plans, the young scientist hopes to follow her dreams and work at the forefront of computer-assisted cancer research and patient care, an easy feat for a hungry mind like hers: ?When you?re passionate about something you?ll be persistent and really enjoy it,? she said.

Though she hardly needs it considering how much she has impacted the world of science already, we wish her all the best in her future career! Congratulations, Brittany!

Depression and Substance Abuse | dailyRx

Depression in patients with access to an opioid prescription can lead to misuse through self-medication. Doctors should screen for depression with chronic opioid therapy patients.

A recent study found that patients on chronic opioid therapy who also have depressive symptoms are capable of misusing their opioids.

The patients in the study had no history of substance abuse, but the more serious their depression?the greater the risk of misuse.

Talk to a therapist if you are experiencing depression.

Mark Sullivan, MD, PhD, professor of psychiatry at the University of Washington School of Medicine, and a team of researchers looked into links between depression and opioid misuse in patients who need the to take opioids for pain management.

The Kaiser Permanente health care group of Northern California and the Group Health Cooperative provided the patient data for the study.

Researchers phone interviewed 1,334 patients with chronic opioid therapy (COT) for non-cancer pain. None of the patients had any history of substance abuse.

Each patient was evaluated for depression and asked three questions:

Did they take their opioid for any symptoms other than the physical pain the medication was prescribed for? Did they increase their doses themselves? Did they give or get opioids from others?

Patients who had depressive symptoms were 1.8 to 2.4 times more likely to misuse opioids by taking them for something other than their physical pain.

Patients with depressive symptoms were also 1.9 to 3.1 times more likely to increase their prescribed dosage.

Authors concluded that there was a link between patients who take opioids for COT who also have depressive symptoms and the misuse of opioids.

They suggested that prescribing health care professionals be aware of depressive symptoms as a risk factor for misuse of opioids, even when the patient has no history of substance abuse.?

This study was published in the July/August issue of Annals of Family Medicine. The study was funded by a grant from the National Institute for Drug Abuse, no conflicts of interest were found.

Depression impacts an estimated 15 million adults in the United States. Depression is a state of prolonged low mood and aversion to activity. A person's thoughts, behavior, feelings and physical well-being are affected and may include feelings of sadness, anxiety, emptiness, hopelessness, worthlessness, guilt, irritability, or restlessness.

The primary treatments for major depression are psychological counseling and medications.

Medication therapies include selective serotonin reuptake inhibitors (SSRIs), serotonin and norepinephrine reuptake inhibitors (SNRIs), norepinephrine and dopamine reuptake inhibitors (NDRIs). SSRIs include: fluoxetine (Prozac), paroxetine (Paxil), sertraline (Zoloft), citalopram (Celexa) and escitalopram (Lexapro). SNRIs include: duloxetine (Cymbalta), venlafaxine (Effexor) and desvenlafaxine (Pristiq). Bupropion (Wellbutrin) is an NDRI. Atypical antidepressants include trazodone (Desyrel) and mirtazapine (Remeron). Each medication category has different side effects.
